CATHOLIC SCHOOL SURVEY

    Catholic School Exploratory Survey

    Holy Family Parish has been given permission to enter into a public exploratory phase of the opening of a Catholic school at Holy Family Parish.  To help us assess this, please indicate your opinion of the statements below. All adults, age 18 years and up are asked to fill out this survey. The survey should take no more than 2 minutes.

    Note about the Indiana state school choice voucher program:  Indiana's Choice Scholarship Program, commonly referred to as the voucher program, provides scholarships to eligible Indiana students to offset tuition costs at participating schools. Students must satisfy both household income requirements and student eligibility criteria to qualify. Recent voucher eligibility expansion means that most families will qualify for Indiana’s Choice Scholarship Program.

    Question 1:  I value a Catholic education.

    Question 2:  Assuming tuition and transportation were not an issue, if I had kids, I would send them to a Catholic school.

    Question 3: Assuming that most of the tuition is covered by the state school choice voucher program, I am willing to put money towards a child’s education (my child or someone else’s child in the form of a scholarship).

     Question 4: I am willing to financially support capital projects at a parish school (building projects, technology upgrades, equipment purchases, etc).

     Question 5:  Holy Family Parish should pursue further the possibility of opening an elementary school in LaPorte
